Hans debut var på Det Norske Teatret i 1955, hvor han jobbet fra 1956 til 1969. Siden var han ved Nationaltheatret fra 1969 til 1975 og igjen i 1985. Han var i Fjernsynsteatret fra 1975 til 1979 og sjef for Telemark Teater i perioden 1982 til 1984. Han huskes særlig for sin tolkning av Vidkun Quisling i Stein Ørnhøis dokudrama «Et liv – en rettssak» fra 1988. Han spilte også i flere norske filmer. Kvalem var forbundsleder for Norsk Skuespillerforbund 1974–1976.
